For one-on-one nutrition counseling:

Please complete the “Getting Started Packet” and “Payment and Cancellation Agreement” prior to our session. If possible, email the forms 48 hours before our session. At the very least, bring your completed forms to our session. This will help ensure that our time spent together is as efficient and productive as possible. Please note, I prefer that you bring in 3 days of food logs. If you have any questions, do not hesitate to contact me.
